Offboarding
You or your supervisor/worklead promptly notify the ESE Operations team of your departure. This notification is necessary to facilitate a smooth lab clearance, which must be initiated by completing the ESE Departure Questionnaire form below.Â

Employees who intend to leave Berkeley Lab should notify their supervisors two weeks or more in advance, if possible, to allow for prompt planning and recruitment of replacements. Submit the resignation notice in writing (letter or email).Â
After your supervisor has acknowledged your resignation and notified Human Resources, you will be contacted with instructions and additional information to guide you through the exit procedure. ESE Ops will get notified automatically. Follow the instructions from the departure email in your inbox.Â
To start your part in the departure process, please complete the documents provided below.Â
Complete the following forms prior to your checkout appointment and send a copy to ESE-Admin@lbl.gov.
When notified by an employee that they plan to resign (including resignations due to retirement), the supervisor must immediately:
Acknowledge in writing the employee’s intention to resign and;
Send a resignation request so Human Resources can initiate the departure procedure.Â
To do so, visit the HR Shared Services page and click the “Initiate Request” (see the image below), then select the “Submit Voluntary Resignation” or "End an Employee's Appointment" and complete the fields provided. ESE Ops will automatically be notified and initiate the offboarding process.Â
Human Resources will provide you with instructions and additional information to take you through the exit procedure when you inform them of the employee's resignation or have reacted to a notification of appointment expiration. This involves providing guidance on releasing the employee's time to ensure final check processing and completing an Exit Checkout Sheet* with the departing employee.Â
When the termination action has been processed in the HR system, you will receive a notification from IT explaining how computers and accounts will be disabled and given the opportunity to request alternatives. Visit here for information on the Termination Notification System (TNS).

Initial Affiliate appointments are generally for up to 12 months but can be extended to another 12 months if necessary. This process guarantees that the nature of the association is examined yearly to see if it is still aligned with company needs and fulfills specified parameters.
Based on the established end date, the Affiliate's host/supervisor will receive an automatic notification questioning whether the appointment will conclude as scheduled or whether an extension is required. It is critical that the host responds to the notification in a timely manner to ensure appropriate processing time for the action (e.g., visa processing, end date planning, etc.) and to avoid additional notifications due to no response.Â
If the affiliate is ending their appointment, ESE Operations is responsible for collecting the affiliate’s LBNL badge, parking permit, laptop, keys, and any other Berkeley lab property prior to their departure. Notify ESE Operations by completing the ESE Departure Questionnaire form (see the Smartsheet form below).Â
Host/Supervisor should contact the employee's home institution's HR office. For ESE purposes, please contact ESE-Admin@lbl.gov to inform whether the appointment is to be extended or the appointment is to end as scheduled to ensure the ESE Operations team prepares the offboarding process.Â

To transition your employee to an affiliate, complete the ESE Employee & Affiliate Request form at least a week before your employee's departure so that the affiliate application can be processed.Â

If you are unable to physically come to the lab to clear out your area and return the items mentioned above, please coordinate with your worklead to be present at checkout on your behalf. See the instructions below:
As per DOE policy, it is required for all departing staff to return all badges
Unreturned Wareham access cards are $25, and unreturned hangtags are $50, payable by you
